I consent to Luxe Care Home Spa processing the health information I give here, which is sensitive personal information under the Data Privacy Act of 2012 (RA 10173), only to check whether a treatment is safe for me and to brief my assigned therapist. Only my therapist and the supervisor on duty can see it. It is deleted 360 days after my session. I can withdraw consent or ask for access or deletion at any time. See our Privacy Notice. I confirm that my health answers are true and complete, and that I'll tell my therapist about any change. I understand that massage is a wellness service, not medical treatment, and that some effects, such as temporary soreness, redness or cupping marks, can occur. I accept these ordinary risks. I will follow the Safety Code. If I'm booking for someone else, I confirm they agree to these terms and the health check, and that they are 18+. Nothing in this acknowledgement limits Luxe Care's responsibility for its own negligence, fraud or bad faith, or any right I have under the law. I agree to the booking & cancellation policy. Full payment is taken online when you book. Full refund if you cancel at least 12 hours before. 3–12 hours before: 75% refund. Under 3 hours or after your therapist has left: a 50% refund.
